Registration Instructions: 

Please take a close look at all the categories below and add together ALL that apply.  On the next page you will be asked to complete a credit card form.  You MUST enter the total amount you have determined from this page and enter it with all of your information.

In the comment box type in Spouse, Additional Guest(s) or Child(s) name(s) and/or any additional information about your registration.

If you are office personnel registering for your doctor, and their name is not the name on the card, please note that in the comment box.

If you wish to pay by check please download the Registration Form at the bottom of this page and mail back to the Society Office with your check.

Cancellation Policy: Full refunds minus a $50 service fee will be considered if written notice of cancellation is received on or before August 15, 2019. No refunds after August 15, 2019
